• If you will be under 18 at the time of the trip, please read the Covenant below and fill out the application form with your parent/guardian. Your parent/guardian will need to insert their electronic signature into the form. The Mexico Mission trip is open to youth who have graduated from 8th Grade (entering 9th Grade the following fall) and older.

    Note: There is one form that must be downloaded, completed, and then uploaded into the form below: the Amor Participant Form.

    We recommend using a scanner to upload these forms, however if you don't have a scanner, you can use an app like CamScanner on your phone or tablet to scan your document.


    Early Bird deadline application forms are due by March 26, 2018 with a $150.00 NON-REFUNDABLE deposit.

    Regular deadline application forms are due April 16, 2018 with a $200.00 NON-REFUNDABLE deposit.

    The total trip fee balance is due no later than May 19th, 2018, the day of the Mandatory Participant & Parent Meeting.

  • Release and Waiver of Liability - Youth Participant


    Preserving the integrity of our Mission is of the utmost importance. Part of that integrity comes from the trust and respect that the directors and sponsors place in the youth to uphold the spirit and guidelines of the trip as well as all U.S. and California laws. However, in order to ensure that our Mission community fully complies with all laws and trip guidelines, the directors and a sponsor from your church may search you and your personal belongings at any time during the trip if necessary.

    THIS RELEASE AND WAIVER OF LIABILITY (“Release”) is executed by the individual on the date input below, the Volunteer or his/her parent having legal custody and/or the legal guardian of the Volunteer (“the Guardian”), in favor of First Christian Church (Disciples of Christ), Concord; its directors, officers, employees, and agents (hereinafter collectively referred to as “the Church” and individually as“Constituents”). The Volunteer and/or Guardian desire that the Volunteer participate in volunteer work projects and the activities related to volunteer work projects in California and Mexico. The Volunteer and/or Guardian understand that the activities may include: building activities, general cleanup, debris removal, repairs, travel to and from the work site, and consuming food and inhabiting living accommodations provided for the volunteers (hereinafter collectively referred to as “Work Projects”).

    The Volunteer and/or Guardian do hereby freely, voluntarily, and without duress execute this Release according to the following terms:

    1. Waiver and Release. In consideration of the Volunteer being allowed to participate in the Work Projects identified by the Church or one of its Constituents, the Volunteer and/or Guardian do hereby release and forever discharge and hold harmless the Church, and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from Volunteer’s participation in the Work Projects. Volunteer and/or Guardian understand that this Release discharges the Church with respect to bodily injury, personal injury, illness, death, or property damage that may result from Volunteer’s participation in the Work Projects. Volunteer and/or Guardian also understand that the Church does not assume any responsibility for or obligation to provide financial assistance or other assistance including but not limited to medical, health, or disability insurance to the Volunteer and/or Guardian.

    2. Medical Treatment. Volunteer and/or Guardian do hereby release and forever discharge the Church from any claim whatsoever which arises or may hereafter arise on account of any first aid treatment or service rendered or not rendered in connection with the Volunteer’s participation in the Work Projects or with the decision by any representative or agent of the Church, to exercise or not exercise the power to consent to medical or dental treatment as such power may be granted and authorized in the Parental Authorization for Treatment of a Minor Child.

    3. Assumption of the Risk. The Volunteer and/or Guardian understand that the Work Projects may include activities that may be hazardous to the Volunteer and that the food, accommodations, and medical facilities may be donated to the Church or to its Constituents and that therefore the quantity,quality and suitability is beyond their control.

    4. Insurance. The Volunteer and/or Guardian understand that neither the Church nor its Constituents carry or maintain health; medical or disability insurance coverage for any Volunteer. Each Volunteer is expected and encouraged to arrive with medical or health coverage in effect.

    5. Other. Volunteer and/or Guardian expressly agree that this Release is intended to be as broad and inclusive as permitted by applicable law. The Volunteer and/or the Guardian stipulate and agree that this Release shall be governed by the laws of the State of California, without regard to the California’s choice of law provisions. Volunteer and/or Guardian agree that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.

    Parental Authorization for Treatment of a Minor Child
    In the event of illness or injury, I authorize the physician and/or hospital to undertake such treatment of and perform such services for the youth as are reasonably indicated by the circumstances.

    The Volunteer and/or Guardian hereby waive any and all rights and benefits otherwise conferred by statutes such as California Civil Code section 1542, which provides as follows:

    “A general release does not extend to claims which the creditor does not know or suspect to exist in his favor at the time of executing the release, which if known by him must have materially affected his settlement with the debtor.”
